Anatomy (Topographic) · MU Sofia

236 midiastaum

Explains the mediastinum as the partition between the pleural sacs, with its boundaries and the horizontal T4 plane that divides it into superior and inferior parts, the latter further split into posterior, middle and anterior sections. Lists the contents of each region, notes structures shared between regions, and describes the vagus, phrenic and aorta in a comparison table. Closes with the course of the thoracic duct and lymph drainage routes.

  14. Lymphatic vessels in thorax

The Mediastinum

  • It is a thick moveable partition between right & left pleural sacs & lungs.
  • It includes all the structures which lie in the intermediate compartments of the thoracic cavity

Boundaries :

  • Superior: Thoracic outlet: (manubrium, 1st rib &T1)
  • Inferior: Diaphragm
  • Anterior: Sternum
  • Posterior: 12 Thoracic vertebrae
  • Lateral: Lungs & pleurae

Divisions

by a horizontal plane from sternal angle to lower border of T4 into:

  1. 1. Superior mediastinum (1 part) : above the plane
  2. 2. Inferior mediastinum (3 parts) : below the plane

Subdivided into:

  • Posterior mediastinum : behind the heart
  • Middle mediastinum : contains the heart
  • Anterior mediastinum : in front of the heart

Level of T4

  • It is the Level of:
  1. 1. Sternal angle
  2. 2. Second costal cartilage
  • Why the Level of T4 is important:
  1. 1. Bifurcation of trachea
  2. 2. Bifurcation of pulmonary trunk
  3. 3. Beginning & termination of arch of aorta

Boundaries

  • Superior: Thoracic outlet
  • Inferior: Horizontal plane
  • Anterior: Manubrium of sternum
  • Posterior: Upper 4 thoracic vertebrae
  • Lateral: lungs & pleurae
