46. Corneal dystrophies

Covers degeneration of the cornea, where normal corneal cells undergo degenerative change with age or disease. It sets out a location-based classification and a cause-based one, noting that the cause-based grouping is the preferred method, then describes age-related forms such as arcus senilis, Vogt's girdle and Hassall-Henle bodies. Pathological types follow, including lipoid, hyaline, amyloid, calcific and Salzmann's nodular degeneration, with their clinical features and treatments, ending with Terrien's marginal degeneration.

Degeneration of the Cornea Topic 29

= conditions when normal cells of cornea undergo degenerative changes due to influence of age or some pathological conditions

Classification

A) Location wise

  • Axial corneal degeneration
  1. 1. Fatty hyaline calcific degeneration (band keratopathy)
  • Amyloidosis Salzmann’s nodular degeneration
  • In Periphery of cornea
  • Arcus Senilis
  • Vogt’s white limbal girdle
  • Hassal-Henle bodies
  • Terrien’s marginal degeneration
  • Moorien’s ulcer
  • Pellucid marginal degeneration
  • Furrow degeneration (senile marginal degeneration )

B) Depending on Cause (which is the preferred method)

  • Age related : Arcus Senilis, Hassal-Henle bodies, Vogt’s, Mosaic degeneration (AHMV)
  • According to pathology
  • Fatty , amyloidosis , calcific , Salzmann’s , Furrow, spheroidal , Pellucid, Terrien’s, moorens

Age related degenerations

1) Arcus Senilis

  • Lipid infiltration(deposits of cholesterol) of periphery , 40-60 common , over 80 =all
  • Usually bilateral (60%), 1mm wide ring

2) Vogt’s white limbal girdle

  • Bilateral chalky white opacities in intrapalpebral area nasal and temporally

3) Hassal-Henle bodies

  • Arise from Descemet’s membrane
  • Outgrowth of hyaline material into anterior chamber around corneal periphery
  • Most commonest corneal changes in elderly
