Pharmacology · MU Sofia

9. Specific and Non-Specific Drug Action & Types of Receptors.

Covers how drugs act through specific targets such as receptors, enzymes, ion channels and transporters, with examples for each, and contrasts this with non-specific actions like membrane effects, osmotic laxatives and direct neutralisation. It then goes through four receptor families: ligand-gated ion channels, G-protein-coupled receptors, enzyme-linked receptors and intracellular nuclear receptors, each with its mechanism and examples. It ends with recap questions.

Specific and Non-Specific Drug Action & Types of Receptors.

Drugs exert their effects by interacting with biological targets in the body. These interactions can be specific or non-specific, depending on how the drug exerts its action.

Specific Drug Action

  • Target-Specific Mechanism: The drug binds to specific cellular targets such as receptors, enzymes, ion channels, or transporters to produce a therapeutic effect.
  • Examples:
  • Receptor Agonists/Antagonists:
  • Morphine (opioid receptor agonist)
  • Propranolol (beta-adrenergic receptor antagonist)
  • Enzyme Inhibitors:
  • Aspirin (cyclooxygenase inhibitor)
  • Sildenafil (phosphodiesterase inhibitor)
  • Ion Channel Modulators:
  • Lidocaine (sodium channel blocker)
  • Nifedipine (calcium channel blocker)
  • Transporter Inhibitors:
  • Fluoxetine (serotonin reuptake inhibitor)

Non-Specific Drug Action

  • Broad Mechanism: The drug does not bind to a specific receptor or enzyme but alters cellular processes or affects the cell environment non-selectively.
  • Examples:
  • Anesthetics:
  • Ether and chloroform alter cell membrane properties, leading to loss of consciousness.
  • Laxatives:
  • Magnesium sulfate (osmotic effect) draws water into the bowel to induce defecation.
  • Antacids:
  • Sodium bicarbonate neutralizes stomach acid via direct chemical interaction.

Types of Receptors

  1. 1. Ionotropic Receptors (Ligand-Gated Ion Channels).
  • Mechanism:
  • These receptors are ion channels that open upon ligand binding, allowing ion flow across the membrane.
  • Examples:
  • Nicotinic Acetylcholine Receptors: Mediate synaptic transmission at the neuromuscular junction.
  • GABA_A Receptors: Allow chloride ions to enter the cell, producing an inhibitory effect.
  1. 2. Metabotropic Receptors (G-Protein-Coupled Receptors, GPCRs)
  • Mechanism:
  • These receptors activate G-proteins, which trigger intracellular signaling cascades.
  • Examples:
  • Beta-Adrenergic Receptors: Increase heart rate and contraction force during the fight-or-flight response.
  • Dopamine Receptors: Involved in mood regulation, reward mechanisms, and motor control.
  1. 3. Enzyme-Linked Receptors (Receptor Tyrosine Kinases, RTKs).
  • Mechanism:
  • These receptors have an extracellular ligand-binding domain and an intracellular enzyme domain. Binding of a ligand leads to protein phosphorylation and cellular responses.
  • Examples:
  • Insulin Receptors: Regulate glucose uptake and metabolism.
  • Epidermal Growth Factor Receptors (EGFR): Involved in cell growth and proliferation.
  1. 4. Intracellular Receptors (Nuclear Receptors)
  • Mechanism:
  • These receptors are inside the cell (cytoplasm or nucleus). The ligand must cross the cell membrane to bind, leading to gene expression regulation.
  • Examples:
  • Steroid Hormone Receptors:
  • Receptors for cortisol, estrogen, and testosterone regulate gene transcription.
  • Thyroid Hormone Receptors:
  • Influence metabolism, growth, and development.
