113. System of portal vein. Porto-caval anastomoses, cava-caval anastomoses.

Covers the tributaries and course of the hepatic portal venous system, including the portal vein, splenic, superior mesenteric and inferior mesenteric veins, with their relations to the pancreas and duodenum. It then sets out the porto-caval anastomoses as four numbered routes linking the portal system to the superior or inferior vena cava, noting where dilated connections can cause varices. Cava-caval anastomoses are not discussed in the text.

SYSTEM OF THE HEPATIC PORTAL

The portal venous system

  • Portal vein
  • Splenic vein
  • Mesenteric veins

neck of pancreas (+)

  • The abdominal part of gut tube, spleen, pancreas, and gall bladder are drained by the portal venous system.
  • The portal venous system delivers blood from these structures to liver to pass through hepatic sinusoids, then to the hepatic veins which return the venous blood to the inferior vena cava just below the diaphragm.

* The lower part of the rectum is not drained by the portal venous system.

Portal vein :

  • Begins : behind the neck of pancreas by union of splenic and and superior mesenteric veins.
  • Ends by giving right and left branches which enter the liver parenchyma.

Portal vein is unique because it begins as vein (form by tributaries) but ends as an artery (gives branches).

- Anterior relations of the portal v. :

  • Infra-duodenal Part : behind the neck of pancreas
  • Retro-duodenal Part : behind 1st part of duode., common bile duct, gastroduodenal artery.
  • Supra duodenal part : It is behind common bile duct and hepatic artery proper.
  • In Porta hepatis : It is the most posterior structure.

Portal vein always shy remain behind other structures.

• ONLY the IVC is behind the portal v.
